Meeting with Dean 20/12/2010

Dear students,
The dean of the university had conducted a meeting on 20th December 2010 (Monday).  The university would like to convey these messages to all students:
  1. Students are reminded to take good care of themselves, especially out in the streets.
  2. Students are expected not to create any forms of attention and conflicts. If any problem arises, students should always refer this matter to the university or curator as soon as possible.
  3. Students are not advised to wonder outside at odd hours. Students are recommended to stay home after dark.
  4. The university assures that the current situation in Volgograd is still under control as enough safety measures have been carried out, despite the riots happening in other cities in Russia.
  5. For students staying in apartments, students are required to obey the rules of their respective premises. Any violation of rules, for example causing fire will end you up with court case and fine.  In general, lowering down the noise production after 10pm.
  6. Students are not advised to leave Volgograd for this coming New Year break. If you need to do so, you must obtain permission from dekanat together with application letter and parent’s supporting letter.

Message from MSA President Chai Chan Sin

Dear Malaysians,

We are glad to announce that the MSA constitution is finally ready. The aim of forming the constitution is to set a guideline for the MSA committee and its members in regards to their responsibilities and conducting an election. It has been made and tailored to suit the system and environment in VSMU.

There are always pros and cons when implementing a new system. We hope that members can be understanding, and give full support to the new constitution. If the system implemented proved to be ineffective, the points in the constitution can be rectified appropriately, as stated in the constitution.

The election will be conducted on the 27th of December 2010. We understand that it is a rush and everything has to be carried out within a short period of time, so we hope that everyone will give their full cooperation to their batch representatives, in order to smoothen and hasten the election process.

For those who aspire to run for a position in the MSA, please do not hesitate to campaign. You may start now by sending the information to the Media Director of Election Committee, Mr. Teng Kok Liang (kokliang85@gmail.com, 89178461691)

Last but not least, I would like to wish everyone a Merry Christmas and Happy New Year in advance. See you all on election day!

Thank you.

President Chai

Summary on Meeting with University ( 10/09/10 )

Practical:

-According to the Dean, Mr Emelyanov, as the pro-rector Prof. Spasov is not in Volgograd, the summer practical rules is set by Russian government, MMC just supported it as they think overseas students should carry out  their summer practical in the respective countries where they are studying in.

-The duration of summer practical as well as when it is carried out cannot be changed because our university is strictly following the standards guideline offered by Russian medical council, this means that both Russians and foreigners students need to carry out this summer practical from 1st until 28th July every year unless otherwise stated by your specific department where you are going to carry out the practical.

-Mr. Emelyanov promises to increase the quality of the practical. Soon a survey list from university will be distributed.

-Meanwhile, MSA also hope that students can fill in the survey list provided in MSA blog, as a feedback for university and Malaysia Embassy.

Registration:
-University cannot change the visa expiry date to 1st of October as requested by MSA, because it is a rule set by Immigration Department of Volgograd. And this rule varies according to different states.

Safety:
-The university reminds all students to take good care of themselves, do not do things that could endanger yourselves.

- Prof. A.A Spasov had showed MSA committee the university’s property and facilities (summer camp) where students may use the facilities for occasions provided with booking procedure and a minimal surcharge. The detail regarding the summer camp will soon be announced.

-Students living in the hostel are required to obey the hostel rules at all times, especially regarding the electrical appliances usage.

Conclusion: In conjunction with this, MSA will write in letter to Ambassador, to have the above issues clarified and finding the best solution which is beneficial to all parties.

Issues Concerning the Obkhodnoi List and Visa

Books taken from the library on Kozolovskaya street can be returned throughout the holidays from 11am to 3pm except on weekends.

Books taken from the main library have to be returned by the 16th of July 2010, on which date it will be closed for the semester.

Therefore, it is advisable to return all books and obtain the librarian signature for the obkhodnoi list before the 16th of July 2010.

The payment for the registration and visa can be done in advance, though the receipt must be kept for processing when students return from their holidays and submit their visa for processing.

The payment for the registration and visa requires different receipts. In other words, you will need one receipt for visa and another for registration for the registration and visa processing. The registration fee for one year [2 semesters] is 600.00 roubles. If you want to pay for one semester, please refer to the Passport-Visa department on the Russian Language department for the amount that you have to pay. The amount you have to pay for visa is 1000 roubles.

For your information the price for these two was set by the Government of the Russian Federation, not the University, and it is the same throughout the country.

Here's how you should fill the payment slip

Take note!
  1. Under сумма write 600 or 1000 depending on whether you're paying for visa or registration. You need to have TWO [2] different receipts, one for visa, one for registration.
  2. Under абонент fill in your full name and country of origin in Russian.
  3. Under адрес fill in  the address of the place where you will be staying. If you're not sure where you'll be staying, it is better not to pay first because you'll need to pay 200 roubles to change the address during the registration itself.
  4. Payment can be made at any Свербанк России branch in Volgograd.
  5. In some of the Свербанк России branches, one only needs to tell the cashier the details, instead of filling out slips.
This slip sample will also be provided upon request from your batch representatives or from the Passport-Visa department on the Russian Language department.

Students must leave Volgograd three [3] days after obtaining their visas from the visa department.

ST elevation in leads II, III, aVF,
ST depression in leads aVL, V1-V5.

Interpretation: Acute inferior infarction, 1st degree block and RBBB.

Spot Diagnosis:
Empyema Necessitans.
It is a spontaneous discharge of an empyema(pus) that has burrowed through the parietal pleural, usually into the chest wall, to form a subcutaneous abcess that may eventually rupture through skin. More commonly secondary to pneumonia.

ECG 2)
Acute inferior myocardial infarction (ST elevation as mentioned, transmural, due to the pathology Q ), with reciprocal changes in V1-3,avL ( ST depression ), RBBB and AV block 1st degree.

Most likely involve left anterior descending artery (LAD)which supplies the base, AV node, RBB, anterior division of LBB ( bifasicular block, which show left axis deviation on ECG )

Spot diagnosis: Pus discharge from fistula as a complication after chest trauma.

Entry No.2

spot diagnosis: site for chest tube (tube thoracostomy) for empyema

Entry No.4Answer for ECG
1.- Sinus rhythm, heart rate 75bpm
- Left axis deviation
- PR prolongation
- Abnormal progression of R-wave in chest leads
- Pathological Q-wave & ST elevation in III, aVF

2.- First degree AV block
- Old infacrt or myocardial scar in inferior wall of left ventricle (LAD, deep Q-wave, QRS fragmentation)
- Possiblity of new acute MI (ST elevation in inferior lead, accompanied by reciprocal ST depression in left lead)
- Left atrial hypertrophy (biphasic P in V1-V2)
- Patient has hypothermia (J-wave) due to AMI
- Possibility of quinidine toxicity (AV block, rollercoaster pattern)

Answer for Spot Diagnosis:
Chylothorax with chyle fistula

Entry No.5
Answer for ECG
1. Sinus rhythm, normal axis, prolonged PR-280ms, broad QRS complex, normal QT, ST elevation in lead II,III,AVF, RSR pattern in V1-V3, Osborne/J waves are noted in V4-V5.

2. Acute inferior myocardial infarction associated with first degree AV block and RBBB.

J wave may be due to severe ischemia etiology.

Answer for Spot Diagnosis:
From the location of the wound and the scar around the wound seen, it is most probably an old scar of chest tube drainage.

Dx: Empyema necessitans

MSA Academic Quiz - Week 1 Result

The winner for the MSA Academic Quiz - Week 1 is...

Drum roll, please...

CHONG KOK HUNG from Group 37, 6th year.

Congratulations, Chong Kok Hung on your winning entry. Your prize will be passed to you by an MSA Committee representative.

Other entries were as follows:

Entry #1

Answer for ECG:
Irregular rhythm. Sinus rhythm.
Heart rate = 60-70bpm
Normal cariac axis - QRS complex is upright in I and aVF, RII>RI>RIII.
RBBB - M pattern in III, aVF, V2. It is due to the occlusion of LAD supply.
Concordance T-inversion in I, V1-4. Biphasic T-wave ---> Anterior NSTEMI, mainly due to occlusion of LAD
Progressive T-inversion in II.
Pointed P-wave in II, III, aVF; no P-wave in I; P-inversion in aVL - possibility of ectopic right atrial focus. Atrial pacemaker having 75bpm

Answer for Spot Diagnosis:
Alcoholic liver disease
Bruises due to:

1. Frequent falling
2. Hemorrhagic syndrome due to reduce of coagulation factor synthesis from liver ( factor I, II, VII, IX, X)

Red bleb = Spider naevi due to reduce of oestrogen metabolism. Oestrogen acts as vasodilators. Note that spider naevi only found above umbilicus

Entry #2
ecg= supraventricular extrasystole/ escape beat

the ecg shows irregularly irregular patern with p before qrs complexs.

spot dx = hemorrahgic teleangictasis@spider neavi
the man probably suffer from alcoholic liver dz complicated with liver cirrosis, hepatic encepalopathy ( freq fall), hypoproteinamia n coagulation fx (bruises). should look for palmar erethema, jaundice, flaping tremor, ascites n other liver sign n portal hypertension.

Entry #3

ECG:sinus rhythm rate 75/min
normal cardiac axis
second degree heart block,2 p wave perQRS
complex,
right bundle branch block pattern,
ST elevation by 2mm in V1,V2-anterior MI
spot diagnosis:spider angiomata(nevus araneus, spider nevus, vascular spider, and "Spider telangiectasia)

Entry #4
ECG: rhythm: irregular
pace maker: SA node (p wave present)
heart rate : 75 beats per min
P-Q interval : 0.12 sec
QRS complex : 0.1 sec
time of internal deflection
Right ventricle : 0.02 sec
left venbtricle : 0.06 sec
p wave : P pulmnonale ( II, III , AVF)
QRS complex : normal
T wave : biphasic
diagnose: Right atrial hypertrophy
Spot diagnosis: spider naevi , in possibly alcoholic liver cirrhosis

Entry #5
Answer for ECG:
Heart Rate: 75
Rhythm: Sinus rhythm
Diagnoisis: Lateral non-elevated S-T segment Myocardial Infarction with Left Bundle Branch Block

Answer for Spot Diagnosis: Nervus Araneus (Spider Nevus)
A spider nevus consists of a central arteriole with radiating thin-walled vessels. Compression of the central vessel produces blanching and temporarily obliterates the lesion. When released, the threadlike vessels quickly refill with blood from the central arteriole. The ascending central arteriole resembles a spider's body, and the radiating fine vessels resemble multiple spider legs. Spider nevus is often found in young adults.
Rapid development of numerous prominent spider angiomas may occur in patients with hepatic cirrhosis, malignant liver disease, and other hepatic dysfunctions due to the elevated level of estrogen hormone.
The patient admits drinking a lot for the pass years suggests a liver disease in this case with the development of this sign.

Entry #6
ECG- Burgada Syndrome
with normal heart rate, sinus
pathological board QRS complex 0.16s
(suspect bundle branch block)
and rsR' /m in lead II,III, aVL, aVF, V1 & V2
and deep wide S in V5, V6
(suspect RBBB)
but RBBB alone, should have ST iso-line,
there is ST Elevation.
(check hemiblock & bifasicularblock; suspect MI)
To check hemiblock or bifasicularblock,
look at lead I, II, III, there is normal axis R2>R1>R3 , not axis deviation thus exclude it.
To check MI by using Sgarbossa Criteria,
all terminal deflection are discordant to T wave, thus is not myocardia infarction (MI).
STE especially in V1-3
is typical sign for Burgada syndrome.
Spot diagnosis- spider navei
patient is chronic alcohol abuse, cause hepatic disease or mostly end with alcoholic cirrhosis.
Spider navei is most common sign of cirrhosis
and it typically localised in upper part of body.
Pathogenesis- Hepatic disease with impaired hepatic function to detoxify estogen in blood, causing high estrogen in blood, and induce nitric oxide synthase in vasoendothelium and increased nitric oxide which lead to vasodilatation.
